Focus is a combat overhaul, with the "focus" on making the player more engaged when in combat. Starfield vanilla has a combat where you can ignore most enemies and most actions, due to many things. The Goal isn't to make combat harder, it is to make it interesting, but something that will come with it. My Goal is to Improve Vanilla, not create some realistic sim.

The Player Character has too much movement freedom compared to the AI, which leads to the issue of being too easy to avoid hits and creatures can't even get near. Now, player speed is reduced by 20% when weapons are draw. Moving backward and to the sides is slower than front, and back is the slowest. Inside combat, boost fuel has a little delay on the regen after use. Sliding goes further and now has some use in combat. Walking speed increased. Oxygen Consumption increased when entering combat.

Vanilla accuracy stats can just be ignored in almost all weapons. Now, It has new recoil patterns to all guns, every one of them has its own identity. It has some easy/safe guns to use, like beowulf, but now most of them have a learning curve or a "power limit". Looking Shotgun's recoil is a nice way to test if this mod is working. "Energy" has patterns... unusual and soft in general. Is It modern, pretty clean, fancy shenanigans? Then it has to have some smooth outcome. Is It hard to understand how to grip and seems painted in some garage? Just be prepared to get surprised in every shot.

Now, Reloading makes the Player Move slower by 20% and In General, you reload faster, but if you move, your reload becomes slower than vanilla. With that, reloading becomes a more tactical decision than some task after some shots.

SunlessSpace nerfed duration from 8 seconds to 5. Time Phase reworked. (Positioning Power) MedKit regen speed got nerfed to add more relevancy to other types of heal. Player deals less stagger damage in general and has a major delay between possible staggers on enemies, but Shooting on legs can almost guarantee a stagger and deal a little less damage. 50% more damage if target is in a stagger animation. Health gains from food are 2x more if it is set to "more" in settings. (Item's description doesn't seem to show the right amount of health) Fitness Improves sneaking speed. While boosting with the boospack, enemies will miss the player's vitals.

Vanilla Difficult settings usually don't matter in land combat due to how Bethesda let players to easily control the AI and how afraid they set their DPS. Focus changed Human AI to be more aggressive, avoid fleeing too soon, have distinct classes, and have more accuracy in general. Elite soldiers will hardly miss shots on you. They will make a little mark on your head. Stay out of sight of Snipers. If an enemy chooses to grab a melee to hit you, they should now be dealing more reasonable damage to you.

The Starfield's Stealth system works really Odd. It is too hard to complete a base without using a silenced sniper. For some reason, a light from a star is weaker than some electric spotlight. Focus made a complete overhaul on this system. The punishment for detecting when moving is reduced, making fleeing from a bad decision possible now. Noise Detection is reduced when hidden. Alerted enemies (Caution warning) will detect a little easier for the player. In General, detected levels build slower and more stable, but decrease slower. It is more forgiven for little misses, but groups will narrow you down if time is controlled wrong. They can start seeing in further distance, but distances will matter more in the buildup. Staying below a light, even in daylight, won't horn the alarms, it just makes it easier to see you. Aiming down with a sniper/scope creates a little glint, making you easier to be spotted at long distances.
