Improve your follower's stealth

Hit any follower or crewmember with this Stealthify Blade and they'll get the following stealth perks:

Their Sneak and Agility stats will be raised to 100
They'll get the player version of rank 4 Stealth
They'll get the player version of rank 2 Concealment
They'll get the Stealth Suit perk

That should make them significantly better at not getting spotted by enemies, but it's still dependent on them not deciding to attack, and any other settings that affect stealth in general.

You'll need to hit every follower that you want to have the above perks, and there is no undo. You will need to hit everyone again after going through unity. The blade will work on any NPC in the AvailableCrewFaction, which should be all properly recruited followers and active crew.
